Our Needs Assessment is a comprehensive tool to help us get to know you and where you might be in need of support. Together we can help to create a plan to assure you receive the help and care you deserve.
Care Coordination
As your Care Manager, we can help coordinate the resources and services you may need. Whether you are looking to downsize to a retirement community or need caregivers to help in your home, we can help. We can coordinate care through an agency and/or private caregivers. Should you decide to move to a retirement setting, we can help with the actual move and downsizing your current location.
Liaison
Let us help you by acting and communicating on your behalf everyone in your circle, including family, physicians and health care professionals so all are all informed and as much a part of your care needs as you desire. We can advocate and speak on your behalf should you run into an emergency situation, a change in condition or hospitalized.
